We are seeking a skilled Home Health Nurse to join our team at Axios International. As a Home Health Nurse, you will be responsible for providing comprehensive nursing care to patients in their homes, focusing on assisting them to achieve optimal health and quality of life.
This role involves working closely with patients, their families, and healthcare providers to develop and implement personalized care plans that meet each patient's unique needs. You will also be responsible for educating patients and their families on proper self-care techniques, medication management, and other aspects of their care.
Responsibilities:
- Conduct thorough assessments of patients' physical and emotional needs.
- Develop and implement individualized care plans that address patients' medical, social, and psychological needs.
- Provide skilled nursing services, including wound care, medication administration, and pain management.

Educational Requirements:
- A Bachelor of Science degree in Nursing (BSN) from an accredited institution.
