Overview:

We are seeking a skilled and compassionate Home Health Nurse to join our team. The ideal candidate will provide high-quality patient care in the comfort of their own homes, working collaboratively with healthcare professionals to achieve optimal health outcomes.

Responsibilities:

  • Deliver comprehensive nursing care to patients in their homes, following physician orders and established care plans.
  • Monitor patients' vital signs, manage medications, and provide education on self-care techniques.
  • Conduct wound assessments, administer dressing changes, and perform general nursing care as needed.
  • Collaborate with patients, families, and caregivers to develop personalized care plans that meet their unique needs.
  • Communicate effectively with healthcare teams to ensure seamless care coordination.

Requirements:

  • BS Degree in Nursing is required.
  • At least 2 years of experience in a healthcare setting, preferably in home health care.
  • Current license or eligibility letter from local health authorities.
  • Excellent command of English.
  • Demonstrated clinical nursing knowledge and experience in providing direct patient care.

Additional Requirements:

  • CPR and/or First Aid certification.
  • Able to travel to clients and partners outside designated regions as needed.
  • Flexible scheduling to accommodate varied client needs.
