The role of the Home Health Nurse at Axios International is to provide comprehensive nursing care in the community setting, focusing on assisting patients to achieve optimal health and quality of life amidst chronic disease.
This position involves the provision and management of effective nursing care to individuals and/or families in the community or through Patient Support Programs. The Home Health Nurse works in accordance with established standards of nursing practices, adhering to local and international rules and regulations.
Key Responsibilities:
- Providing skilled nursing services in accordance with physician orders and a plan of care.
- Telephonic follow-up consultations.
- Monitoring vital signs during home care, including blood pressure, heart rate, temperature, and oximeter statistics.
- Monitoring blood glucose levels and providing teaching and support for patients and their families.
- Preparing medications as per physician order and administering them according to instruction.
- Pain Management: monitoring and supporting patients with acute or chronic health conditions.
- Education and counseling, consistent with the type/nature of treatment and/or care management need.
- Wound assessment and dressing changes.
- Providing general nursing care.
Educational Background and Experience:
- A BS Degree in Nursing is required.
- At least 2 years of working experience in a healthcare setting.
- Must provide evidence of local health authority license or eligibility letter.
- Excellent command of English.
- Demonstrate clinical nursing knowledge and experience in providing direct patient care.
